Our Story

Elevate Dentistry is led by Dr. Maja Slowik and Dr. Michael Hoffman. It was founded on the principles of integrity, innovation, and personalized care. We proudly serve the Frankfort community and surrounding areas, offering comprehensive, high-quality dental care for patients of all ages. Whether it’s a child’s first visit, an adult seeking cosmetic or restorative solutions, or someone needing emergency care, we’re here to make every experience positive, comfortable, and rewarding. MAJA SLOWIK, D.M.D.

Dr. Slowik was born in Warsaw, Poland. She came to the United States as a young girl alongside her mother, both determined to pursue the American Dream. This experience instilled in her a strong work ethic, a sense of gratitude, and a desire to make a meaningful impact in her community.

Her academic path reflects that drive as she earned her undergraduate degree in Molecular and Cellular Biology from the University of Illinois at Urbana-Champaign, where she also minored in Business and Chemistry. These studies built an ideal foundation for both the science and operative sides of dentistry. She then went on to graduate in the top 15% of her class from Midwestern University College of Dental Medicine in Downers Grove, IL, where she honed her clinical skills and developed a patient-first philosophy that continues to guide her practice today.

But her story doesn’t end with professional achievements. Outside the office, she’s a proud mom to two energetic and imaginative boys, Oliver (4) and Theodore (1). Together, they live in the West Beverly neighborhood of Chicago, where family life is filled with laughter, creativity, and cherished traditions. Whether she’s cooking a homemade meal for a cozy family dinner, traveling through Europe to explore new cultures, or diving into a good romance or thriller novel, she finds joy in the little moments that build lasting memories. Her love for painting, DIY projects, and LEGOs reflects the same creativity and attention to detail she brings to her dental work. These hobbies aren’t just pastimes, they’re expressions of the care, precision, and thoughtfulness that define her approach to patient care.

MICHAEL G. HOFFMAN, D.D.S.

Dr. Hoffman is a graduate of Indiana University (BS Microbiology), and Loyola University School of Dentistry (DDS). He regularly attends seminars and is an active member of the American Dental Association, the Illinois State Dental Society and is a past president of the South Suburban Branch of the Chicago Dental Society.

Married to his wife, Bernadette ("Bernie"), for more than 35 years and a life-long resident of the South Suburbs; (an Eagle Scout) he enjoys hiking, birding, appreciating nature and continues to be involved in ministries at St. Veronica Parish.

He looks forward to assisting patients achieve and keep their beautiful smiles. Compassionate, conservative and creative dentistry is his goal.

While we are out-of-network with all insurance providers, patients are still able to utilize their benefits with minimal difference from in-network care.

Our team will carefully review your individual plan and provide a clear understanding of your benefits and anticipated out-of-pocket costs before treatment. Whenever possible, we obtain pre-treatment estimates to ensure transparency and eliminate unexpected expenses. If needed, we are also happy to discuss flexible financing options to support your care.

We understand that dental insurance can be complex, therefore we will handle all insurance claim submissions on your behalf for a smooth, stress-free experience.

Remaining out-of-network allows us to prioritize what matters most—exceptional patient care. This means more dedicated time with your doctor, experiencing a highly personalized approach, and the comfort of seeing a consistent, familiar team at every visit.

Please note that we do not participate with HMO or public insurance plans, including Medicaid and HIP, as these plans require in-network providers.

Are your procedures painful?

How often do I need to change my toothbrush?

It is essential to change your toothbrush once every three or four months. Always select a toothbrush with soft or extra-soft bristles.

Manual vs Electric Toothbrush?

If you are debating between which type of toothbrush to use, electric offers several benefits over manual toothbrushes. One of the major advantages of electric toothbrushes is their superior cleaning power. Their fast-rotating and vibrating bristles enhance plaque removal which can minimize the onset of tooth decay and gum disease. Many electric toothbrushes also have built-in features like timers and pressure sensors to help you brush properly and for the recommended amount of time (2 minutes or 30 seconds per quadrant). The pressure sensors alert users if they are applying too much force, helping to protect against gum recession, wear of tooth enamel, and tooth sensitivity. Manual toothbrush users commonly and unknowingly brush aggressively thinking that harder equals cleaner teeth. Electric toothbrushes are suitable for all ages, and they make brushing easier and more thorough, promoting better oral health outcomes.
